# Provider Verification Evidence-Only Implementation Plan

**Goal:** Remove generic verification metadata from the provider portal and submit only the verification type plus one or two evidence files.

**Architecture:** Keep the API contract untouched and narrow the browser-side view model and upload builder. Blade remains a thin localized view; JavaScript creates an allowlisted multipart payload.

### Task 1: Remove verification metadata from UI and payload

Assert that neither locale renders metadata labels or input names, that `providerVerificationInput()` returns only `type` and `evidence`, and that the multipart body has no `metadata[...]` fields even if an injected input object contains them.

Run `php artisan test --filter=ProviderPortalVerificationUiTest` and `node tests/js/provider-review.test.js`. Expected: failures caused by the existing metadata controls and multipart field.

- [x] **Step 3: Apply the minimal implementation**

Remove the two Blade controls and translation keys. Remove metadata parsing and validation from the portal JavaScript, and build the multipart body only from the selected verification type and evidence files.

- [x] **Step 4: Rebuild and verify**

Run `npm run test:provider`, `npm run production`, `php artisan architecture:audit --strict`, and `php artisan test`. Expected: all commands exit successfully.

Stage only the files in this plan plus the compiled provider bundle. Use the version hook to evaluate the patch increment and commit with `fix(provider): simplify verification evidence form`.
